How do I participate?

Before participating, read the Guidelines – PDF

Submit the following:

  1. Completed application/expense form - PDF 
  2. Original detailed itemized receipts for travel and accommodations
  3. Confirmation from the Undergraduate Medical Education office providing:
    • Confirmation that you are in good standing;
    • Rotation location and dates upon completion of your rotation;
    • Academic year for the eligible rotation;
  4. Social Insurance Number form - PDF
  5. Confirmation of Banking Information - PDF

Please review the expense guidelines before booking travel and accommodations to ensure all guidelines are followed.

Submit appropriate supporting documents and retain a copy of all the documentation for your records.
